INCIDENT FORM

This form is to be filled out for any incident that occurs within Saskatchewan Minor Softball League to include:

• Ejections from any games or tournaments
• Includes of travesty of the game or incidents that are detrimental to the game of softball. IE: players, coaches, umpires or parents

Insurance Claim

Forms can be picked up from the SMSL office and to be submitted to Saskatoon Minor Softball within 7 days of the accident.

ADOPT AN UMPIRE CLAIM SHEET

Coaches to claim money for either a team or a umpire from Saskatoon Minor Softball League these are the steps that are needed.

• Print a claim sheet and please make sure the stubs (bottom of the scoresheet) are stapled to claim sheet.
• Hand into the office no later than July 15 of current season.

Emergency Action Plan

Athletic Medical Information Form

SMSL recommends that all teams keep an Athletic Medical Information Form in their Coach's / Manager's kit which should be at every game and practice.
